TO APPLY FOR A FIRST-TIME U.S. PASSPORT FOR A MINOR UNDER 16 YEARS OF AGE
You must make an appointment for your application to be processed.
You must PERSONALLY present ORIGINALS off the following documents. If you do not have some of the documents listed below, please be prepared to explain why. You must also present a COPY of the documents indicated by an asterisk (*); otherwise, you will be charged one dollar for each copy.
- Completed form DS-11 (in English, typewritten or printed in black or blue ink; white-out is prohibited)
- One front-view individual color photograph, 2 x 2 inches in size, printed on thin paper with a light background
- Proof of U.S. citizefnship, such fas a certified copy of a birth certificate (long form, not abstract), naturalization certificate, certificate of citizenship, or consular report of birth abroad*
- Hospital record of birth*
- Immunization record*
- If baptized, baptismal certificate*
- Any passports or other identity documents previously issued to you (U.S., Mexican or others)*
- Evidence of continuation of identity: school, photos, yearbooks, etc.*
- Pre-natal medical record for the mother during her pregnancy, including any lab tests, ultrasounds, X rays, photos, etc.
- Any documents or papers which show that the mother was in the U.S. at the time of birth, including hospital records
- Applicants under the age of 16 must be accompanied by BOTH parents (please read the NOTE below).
- Certified copy of birth certificates of all siblings, including half-siblings
- Certified copy of birth cfertificate of father and mother*
- Valid identification for both parents*
- If married, marriage certificate of parents (for marriages)*
- If divorced, divorce certificate of parents (for all divorces)*
- If no longer alive, death certificate of parents*

$35.00 U.S. dollars for passport card

NOTE: If only one parent accompanies the applicant, one of the following must also be submitted:
1. the other parent’s notarized written statement in English (DS-3053), consenting to passport issuance for the child, with a copy of the other parent’s official photo ID;
2. evidence of legal authority to apply without the consent of the other parent (e.g., judge’s order awarding sole custody); or
3. a written statement, made under penalty of perjury, explaining the other parent’s unavailability (DS-3053).
APPLICANTS ARE ENCOURAGED TO PERSONALLY COMPLETE THEIR FORMS AND GATHER ALL THE NECESSARY DOCUMENTATION. IT IS NOT NECESSARY FOR YOU TO PAY FOR PROFESSIONAL ASSISTANCE.
