TO APPLY FOR A RENEWAL U.S. PASSPORT FOR AN ADULT (16 YEARS OF AGE OR OLDER)

You must make an appointment for your application to be processed.

You must PERSONALLY present ORIGINALS of the following documents. If you do not have some of the documents listed below, please be prepared to explain why. You must also present a COPY of the documents indicated by an asterisk (*); otherwise, you will be charged one dollar for each copy.


  • Completed form DS-82 (in English, typewritten or printed in black or blue ink; white-out is prohibited)

  • One front-view individual color photograph, 2 x 2 inches in size, printed on thin paper with a light background

  • Proof of U.S. citizenship, such as a certified copy of a birth certificate (long form, not abstract), naturalization certificate, certificate of citizenship, or consular report of birth abroad*

  • Any passports or other identity documents previously issued to you (U.S., Mexican or others)*

  • Evidence of continuation of identity: school records, photos, yearbooks, etc.*

  • A marriage certificate or court order if your name has changed*

  • The amount of: $75.00 U.S. dollars for passport book
                             $20.00 U.S. dollars for passport card 

    PLEASE BRING exact amount of cash in one type of currency, or credit card.

    THIS FORM MUST BE PRESENTED WITH YOUR DOCUMENTATION. TO PRINT IT IN CHECKLIST FORMAT, PLEASE CLICK HERE.

    You may be asked to provide additional documents once your application has been reviewed.

    NOTE: You can use form DS-82 only if you can answer “yes” to ALL of the following:
    1. you can submit your most recent U.S. passport;
    2. you were at least 16 years old when your most recent U.S. passport was issued;
    3. your most recent U.S. passport was issued less than 15 years ago; and
    4. you are using the same name as on your most recent U.S. passport, OR you have had your name legally changed and can submit proper documentation to reflect the name change.
    If your answer is “no” to any one of the above, you MUST use form DS-11 instead, and pay $100.00 U.S. dollars.

    APPLICANTS ARE ENCOURAGED TO PERSONALLY COMPLETE THEIR FORMS AND GATHER ALL THE NECESSARY DOCUMENTATION. IT IS NOT NECESSARY FOR YOU TO PAY FOR PROFESSIONAL ASSISTANCE.

    U.S. citizens who live at the southern end of Baja California Sur may apply at the U.S. Consular Agency in Cabo San Lucas. Click here for the Consular Agency contact information.
    If you apply at the Consular Agency, you must still comply with all the requirements listed on this site.
